Put 1 tablet under the tongue during chest pain for every 5 minute until the chest pain resolve or when there are side effects associated with the drug.

Vioxx Story- A story that worth sharing

This is one of the subtopic of my presentation titled “How drug company misled healthcare professionals and harm patients” during my DPIS attachment. One of the famous event happened in drug history. The story is quiet lengthy sorry i tried to summarized it already 

Vioxx (rofecoxib), a selective NSAID that made a annual sales of 2.5 billion in 2003 and estimated 80 millions people were prescribed with vioxx at some time and the second famous cox 2 inhibitor next to Celebrex. (celecoxib). On the other hand, this drug also accounted to approximately 90 thousand American’s heart attacks and 38 thousand of death.

Well, although it is very scientific (I dislike to be scientific xD), I would try to explain the story in a more easy way.

The story begin in 1999 when the FDA approve vioxx, in the same year, Merck (Company of vioxx) conducted a study call vioxx gastrointestinal research study (VIGOR), this study involve 8 thousand patient to compare the safety of vioxx to naproxen in gastrointestinal.

Few months later the safety panel review that vioxx have serious heart problems and death among vioxx patients is twice as high as the naproxen group, the safety board votes to continue the trial as the effect might be because of the cardio protective effect of naproxen.

In year 2000, merck get the result of the VIGOR trial and publish it to the New England Journal of Medicine (NJEM), however, they only submitted 17 of the 20 heart attack without acknowledge NEJM the 3 extra death.

7 month later, Merck submitted the result to FDA but still didn’t submit to NEJM, so the study published in NEJM doesn’t mention the another 3 death and leave out alots of data on cardiovascular effects.

(*Note: The data were published by NEJM in 2005 after the withdrawal of vioxx, the relative risk cardiovascular event increase from 4.25 to 5%.)

Then in 2001, some researcher manage to get the data from FDA and made a meta analysis on it, the result cast a serious doubt on the hypothesis that naproxen protects the heart.

The story continue until 2004, there are several epidemiological studies shown vioxx increased cardiovascular problems, but this doesn’t affect the sales of vioxx, and it continue to rise until 2003 for its peak sales.

In 2004, merck voluntary withdraws vioxx after a study called “Approve” which shown that the drug rises the risk of heart attack after 18 months.

By the time it withdraw, it is estimated there are 20 million Americans have taken the drug, research later published in lancet which estimate that approximately 90 thousand American had heart attacks from taking vioxx, and 38 thousand of them died!

So, the next question is, how about other cox 2 inhibitor? Bextra which is marketed by Pfizer were removed from market due to the concern of possible increased risk of heart attack and strokes, which is after vioxx events. Arcoxia, which is the next generation of merck cox 2 inhibitor are not approved by FDA due to insufficient safety data provided.

Next is celecoxib, the celecoxib long term arthritis safety study (CLASS) did not present an increase in events, but however, some researcher cast a doubt on the safety of Celebrex as the CLASS trial is only range from 6 month to 13 month (Note: Vioxx rise the risk of heart attack after 18 month), so Pfizer promise to do a big, large trials for Celebrex named PRECISION trial, which begin in 2006 and completed in may 2014 which is after the Celebrex patent end.

Amlodipine

Usage:

This medicine is used to treat/lowered high blood pressure. This medicine also used to treat chest pain or pressure.

How to use it / Best taken:

Take this drug at the same time of the day; take as you have been told, even if you are feeling better. Take with or without food, take with food if it causes an upset stomach.

Precautions  /Warning:

Have your blood pressure and heart rate checked often. Avoid grapefruit and grapefruit juice. Tell your doctor if you have liver disease or you are pregnant, plan on getting pregnant or breast feeding.

How does this drug work?

This medicine calms blood vessels, lowering blood pressure. It also helps the heart work better

What to do if I miss a dose?

  • Take a missed dose as soon as you think about it. If it is close to the time for your next dose, skip the missed dose and go back to your normal time.
  • Do not take more often than every 24 hour unless told to do so by your doctor.
  • Do not take 2 doses at the same time or extra doses.

Side effects of this medicine:

  • Dizziness – it can be reduce by rising slowly over a few minutes when sitting or lying down.
  • Flushing.
  • Headache
  • Swelling in the arms or legs

Esomeprazole (Capsule)

This medicine is used to treat syndromes caused by lots of stomach acid. This medicine also used to stop or treat ulcers of the swallowing tube (esophagus) and to stop or treat gastrointestinal ulcers caused by infections.

How to use it/Best taken:

Take 1 hour before the first meal of the day. Swallow whole capsule, do not chew, break, or crush. You may sprinkle contents of capsule on applesauce

Precautions/Warning:

Use with care if you have risks for osteoporosis such as alcohol use, cigarette smoking, family member with osteoporosis, taking drugs to treat seizures, taking steroids. If you have liver disease, talk with your doctor.

Usage:

This medicine is used to put off or treat soft brittle bones (osteoporosis) in postmenopausal women. This medicine also used for treatment of osteoporosis in men to prevent fracture

Precautions/Warning:

Talk to your doctor/pharmacist if you have high calcium level, gastrointestinal disease, kidney disease, pregnant, getting pregnant, breast feeding or you are on a low salt diet (this medicine has salt). Do not take cholestyramine, mineral oil, antacid that have magnesium or orlistat with this drug.

How to use it/Best taken:

  1. Choose the day of the week that best fits your schedule. Every week, take one Fosamax tablet on your chosen day.
  2. Taken in the morning at least 30 minutes before foods, drink  or other medications (empty stomach)
  3. Swallow your Fosamax tablet with a full glass of plain water. Avoid taking with mineral water or other drinks.
  4. Swallow tablet whole, do not chew, break or crush.
  5. Do not lie down for at least 30 minutes after taking this drug.  Stay fully upright (sitting, standing or walking), this stops irritation to the swallowing tube (esophagus).
  6. After swallowing the tablet, wait at least 30 minutes before taking your first food, drink or other medications of the day. This will maximize the absorption of medication and can prevent any drug-food or drug-drug interaction.
  7. Do not take tablet at bedtime or before getting up for the day.
  8. If you develop difficulty or pain upon swallowing, chest pain, or new or worsening heartburn, stop taking this medication and call your doctor.

How does this drug work?

This medicine contains 2 drugs:

  • Alendronate lowers bone loss.
  • Cholecalciferol acts as a hormone to control how much calcium is stored in the body and calcium make the bone strong.

What to do if I miss a dose?

  • If you miss a dose, take only 1 FOSAMAX PLUS D tablet on the morning after you remember. Do not take 2 tablets on the same day.
  • Continue your usual schedule of 1 FOSAMAX PLUS D tablet once a week on your chosen day.
  • If you think you took more than the prescribed dose of FOSAMAX PLUS D, drink a full glass of milk and call your doctor right away. Do not try to vomit. Do not lie down.

Side effects of this medicine:

  • Belly pain or heartburn
  • Upset stomach, many small meals, good mouth care, sugar-free candy or chewing sugar-free gum may help
  • Hard stools (constipation) – Drinking more liquids, working out, or adding fiber to your diet may help. Talk to your doctor about a stool softener or laxative
  • Loose stools (diarrhea)
  • High calcium level, sign include weakness, confusion, feeling tired, headache, upset stomach and throwing up, constipation, or bone pain.
  • Rare side effects included:
  • Bone, joint or muscle pain
  • A broken leg
  • Harm to the jaw bone

Storage:

  • Store at room temperature, 20°C to 25°C.
  • Keep away from light.
  • Keep package and tablets dry.
  • Store in the original package.

How to use it/ Best taken:

Take this medicine before food. If you have been told to take 3 times a day:  the optimal dosing time will be 8am, 1pm, 6pm (5 hour gap). If 2 times a day: 8am, 3pm (7 hour gap) to avoid tolerance to medicine and thus reduce its effectiveness.
